Not sure if it's still occurring or is relevant to the thread but when I was employed was addressed by my Surname alone a couple of times - no Mr or other honorific, just my surname.
Apparently one girl complained to her manager who rang me to ask why I'd put the 'phone down on her, he listened to my answer which was to tell him I'm not being addressed like a school boy by anyone at any time, which was when I was last addressed in that form, and when his secretary learns how to address me correctly I'd be happy to reply. He apologised, he also expressed his concern as he didn't know that his secretary was addressing people in that method and that he would explain to her that people were unhappy by being addressed by their surname alone. He did say that he would not like to be addressed by his surname alone.
